How do i put a grade i made up in summer school on my uc and cal state applications?

i made up a math grade....i was wondering how to report it


i got a d/d my sem1/sem2 of 11th grade


this summer before senior year i got c/c....


so how do i report the second grade on my applications


would it b my summer session for my 11th or 12th grade year for cals states....do i even need to report it to cal states...and ucs i know its a must...can anyone please answer this...its that last touch i need tonight next couple hours....How do i put a grade i made up in summer school on my uc and cal state applications?
UC/CSU do not like D's at all, i recommend retaking it.. or something idk
